Do you qualify for an Enhanced Primary Care Plan (EPC)?

What is an EPC?

An EPC is a care plan for a chronic condition that allows you to access 5 sessions in a calendar year with an allied health professional referred by your GP. Medicare subsidises these sessions.

How do you qualify?

If you have a chronic condition which is defined as an injury, disease or illness that has lasted beyond 6 months.

 A chronic illness can be any but not limited to the following:

  • Diabetes
  • Cancer
  • Arthritis
  • Osteoporosis
  • Chronic Back Pain
  • Heart disease
  • Obesity
  • Stroke

Our Exercise Physiologists at Longevity have experience with these care plans and building a program for each individual client and create the best plan with the other Allied Health Professionals involved

How can Exercise Physiology be important in your care plan?

At Longevity Exercise Physiology, all our Accredited Exercise Physiologists are qualified to treat, manage and prevent chronic illnesses. A care plan can involve multiple Allied Health Professionals including a Physiotherapist, Podiatrist and an Exercise Physiologist to ensure you get the best possible treatment for your condition. Our Exercise Physiologists at Longevity have experience with these care plans and building a program for each individual client and create the best plan with the other Allied Health Professionals involved.

I think I may qualify, what now?

Book a consult with your regular GP and have a chat on how you can qualify with your chronic health condition. Your GP will write you a plan and refer you to the appropriate Allied Health Professionals.
