What is a chronic disease management plan?

A Chronic Disease Management plan (CDM) or Enhanced Primary Care plan is to help people with chronic diseases have access to allied health services. Your doctor will work with you to help coordinate the care you need to manage your disease and enable you to have the best health outcomes possible. The care plan allows you up to five sessions per calendar year, which can be divided up between whichever allied health services you need, including:

  1. Find out if you qualify as someone with a chronic condition. A chronic condition is any illness, disease or injury that has lasted beyond 6 months.
  2. Ask your GP if they can create a chronic disease management plan or an enhanced primary care (EPC) plan to help you manage your health.
  3. Request for it to include exercise physiology or any other therapies your doctor recommends.
  4. Bring your EPC referral with you on the day, so you can claim back your Medicare rebate on the spot.

 

Why should I get a chronic disease management plan?

Even though for most specialties, they won’t be enough on their own to get you all the help you need, since you get up to five sessions per year, they are a great way to have a taste of therapies that might be beneficial, including exercise physiology. These sessions are also subsidised by Medicare, and are either bulk-billed, or you will have a small gap to pay.

For exercise physiology, the five sessions will give you more of a preview into how exercise can help you manage your disease and improve your health. Since these sessions are subsidised, they are a great way to find out how exercise can help, and to get you started on your exercise journey!
