How to Get a Chronic Disease Management Plan Referral 

Starting is easy! Simply visit your General Practitioner (GP) and talk about your health goals and challenges. If you have a chronic condition like diabetes, arthritis, or heart disease, your doctor may create a Chronic Disease Management (CDM) Plan. This plan lets you access up to 5 subsidised sessions with allied health professionals, including exercise physiologists. After your GP’s referral, you can book your first session with us and get started on your journey to better health!
