How to Access My Aged Care Packages

Not sure how it all works? Here’s a simple step-by-step guide to accessing My Aged Care and using it for services like Exercise Physiology:

Step 1: Register with My Aged Care

Call 1800 200 422 or visit www.myagedcare.gov.au
You’ll go through a short eligibility check and be referred for an in-home assessment.

Step 2: Get Assessed

A representative from the Aged Care Assessment Team (ACAT) will visit you at home to discuss your needs. They’ll determine your eligibility for:

  • Home Care Packages (HCP) – for more ongoing care
  • Commonwealth Home Support Program (CHSP) – for entry-level services

Step 3: Choose a Provider

Once you’ve been approved, you can choose a Home Care Package provider (if using HCP) or work with services approved under the CHSP.

Let your provider know you’d like to use your funding to work with Longevity Exercise Physiology.

Step 4: Get Started with Longevity

We’ll work directly with you and your case manager to organise:

  • Your initial assessment
  • Ongoing weekly sessions
  • Progress tracking and support to keep you on track
